We work with most major insurance providers except Kentucky Passport, United Healthcare, and Cigna Healthcare. Maintenance appointments are self-pay.
What are your office hours?
Monday, Wednesday, and Thursday 7:00am-1:00pm & 3:00pm-6:00pm; Tuesday 9:00am-1:00pm. Closed weekends.

New patients can schedule online or by calling (502) 231-4003. Existing patients use our app for maintenance appointments.
